Which one is correct or more suitable? Or any suggestion for improvement?

Follow-up consultations, usually at a space of 20 days or a month, are charged at Rs. 100/- per visit.

Follow-up consultations, usually after a gap of 20 days or a month, are charged at Rs. 100/- per visit.

Follow-up consultations, usually after 20 days or a month from the preceding consultation, are charged at Rs. 100/- per visit.
