How to put the sentence correctly?

Hello English teachers and not teachers!

I am writing an assignment (something like a report) in English. The starting sentence should be:
The report is based on the case-study of the instance happened at the company...

I don't like the word "instance" here but I don't know how to paraphrase it correctly. It sounds as if it is superfluous here...
Could you help me please.

It's a strange sentence, it doesn't really mean anything. Should it perhaps be; The report is based on the case-study of the incident that happened at the company...?

I definitely think she was confusing the word "instance" with "incident".
"incident" would be right there.

I treat the words instance and incident as synonyms with the meaning "case". I looked them up in the English-Russian dicionary. I agree, "instance" does not really fit this sentence, but I did not know why. Now I think I understand the difference between the two words better.

If you have written something like, "The Incidence of industrial accidents is rising..." and your sentence is your first reference to a particular incident in a particular factory, then the sentence would be:

If you have NOT referred to the "case-study" prior to this sentence, then the sentence would be:
The report is based on a case-study of an incident that...

If your sentence is the very first of what you are writing, and again, the first reference to the case-study being referred to in your report, then you would also use the amendments in blue.
