Suppose you are doing a course because you want to be a guide. After a successful exam can I say:
I passed an exam as a guide?
(because if you pass an exam AS a guide, it means that you are already a guide - before pasing the exam)

I don't think this is a standard expression in this context: '...as a guide'; because the information is not clear. Though in informal speaking such expressions may be possible.
I suggest:
"I've passed the exam; I'm now a guide."
OR
"I am a guide now; I've passed its exam successfully."

someone may suggest a better one!

How about this: I've pass the guide exam.
