Is the above saying "measures for saving or reducing costs"? No, contraints denote restraints which have a nagative impact on the outcome of any thing (be it be Health Care, Scocial service etc) in terms of both quality and cost. These contraints can be social, political or resourse based.

Does this refer to "small groups of uninsured"? Yes, more or less so. Subset is a set within the entire/a large set. Here it means a small group within the entire group of uninsured.
