    You can say:
    I've had a severe headache for two hours meaning your headache started two hours ago, and you are still suffering from it now.

    In the past, you could say:
    I'd had a severe headache for two hours meaning your headache had started two hours before, and you were still suffering from it at the time.

    It's the past perfect form of "to have".
