like pulling teeth —used to say that something is very difficult and frustrating
▪ Getting him to make a decision is like pulling teeth. [=it is very hard to get him to make a decision]
Can I say "It is like pulling teeth getting him to make a decision"?

Yes, you can. It's an expressive idiom so you are very frustrated or it's very difficult.

You certainly can, but I'd change the order:
Getting him to make a decision is like pulling teeth.
