Sentences like the above are always headaches. Can the word "centered" be linked with "with phrase"? What does the writer want to say here anyway?

This is not actually a sentence since it lacks a main verb. I've modified it above. Does that help?
Other ways of putting this would be:
"Assertiveness is based on a sense of being centered/grounded, accompanied by a nourishing source of personal energy."
"Assertiveness is based on a sense of being centered/grounded, which provides a nourishing source of personal energy."
