Please help me to check my answer to this exercise

Word forms

They are all correct except 5 which should be 'unaccustomed'.

I think # 4 is a red herring in this excercise. I think the word should be unsuccessful.

Successive indicates something happening in quick succession, as in shots being fired from a gun. The fact that the sentence says that he finally passed the driver's test, indicates to me that there was a lengthy time period between the tries.

continuously
I would prefer continually but many people would use continuously.

Yes that's how I see it, but they are often used interchangeably.
