It probably means "the best or the worst'" or "the top or the bottom."

As additional information, you will often hear "On a scale of 1 to 5" or "On a scale of 1 to 10" how would you rate... ?

Usually the high number is very good, or the most, and the low number is very bad, or the least. So something at "one end of the scale" would be a 1 or 10 - either very good or very bad.

It's no always good/bad though. It could be simple/complex, or introverted/extroverted, as just two examples of where neither end is better or worse.
