Good Basic Teacher lesson plan templates

I'm a teacher looking for a good/basic lesson planning template so I can modify and add to Word doc. I don't want to plagiarise it but I'm not great with Excel or Word and trying to make it look official.

A heads up on a link or attachment would be great.
