    It should be 'what had happened', but it makes sense to me.

    The first part ("Although the pilot was badly hurt") says to me that being badly hurt should have helped the pilot to explain what had happened. That doesn't make sense to me, and if anything it should be the opposite. Being badly hurt would tend to "shake somebody up" and make recall more difficult.
