would the reader take offence to this message

Dear all,

Hi, I am new here so please go easy on me!:)

My question is this; could the reader possibly take offence to the message written below, does it sound too harsh or is it an acceptable message if one is trying to put across the importance of what I am asking.

"Dear Mr Bloggs,

We have recently received some paperwork, however, you have neglected to submit any results.

Please could you e-mail them to me at your earliest convenience.

Thank you."

OK, so is the word neglected too harsh or is the use of the word acceptable in the context I have used it, without sounding rude and making an insinuation that the recipient of my e-mail has purposely neglected to enclose the results for which I am asking ? a similar word I could of used was 'omitted', but I am pretty sure the words have similar meanings, if not the same. Can anyone clear this up for me, thanks

"you have neglected" may sound too acusatory.


suggestion:

We have recently received some paperwork, however, the necesary results were not included. Please submit the information to us as soon as possible.
