PLEASE can you help?!

I am working on a project about self evaluation for the TESOL Diploma. I would really appreciate it if any experienced teachers could spare a couple of minutes to complete the attached short questionnaire (8 questions). It is very quick!

I shall send the results to each person who replies to my questionnaire.

Quick question!

I'm happy to fill in your questionnaire but it opens as a simple Word document and I am not clear on what you want. Do you want us to write YES or something next to the letter that represents our answer? Are the dotted lines for comments? And when it's completed, what do you want us to do with it - email it to you (email address required)? Attach it to a post here for all to read?
