I am not an English teacher but I can comment.

    B is not correct.

    Although you could say:

B I should have gotten him to do it.

Or, "I should have got him to do it".

I have the impression that "make" can only be followed by "to" in passive voice. Is this correct?

    My immediate answer is, "Yes".

    I made him do it.
He was made to do it.

I'll come back if further reflection makes me modify that answer.

Where did that come from, bhai? That is not English as I know it.

Uh-oh. Re-thought. I made them (these ginger cakes) (in order) to (be able to) exhibit (them) at the agricultural show.

    How am I doing?

They were made to exhibit at the agricultural show by me. A great many pigs were exhibited and later eaten. Fun was had by all.
