Teacher, can I say this?

I still have 5 vacation days left and I would like to take 2 days off work and the days are June 28 and 29. I need to attend to some personal matters. I hope you will approve my request.

I suggest just a small change.

I agree with bhaisahab that there are too many "and"s in the original. I also don't see the need for the word "work". Presumably, this request is going to your employer so you could just say "I would like to take two days off - June 28th and 29th". You could even shorten it more with "I have 5 vacation days remaining and I would like to take two of them on June 28th and 29th. [The rest is fine.]"
