Can it have both meanings like "watching a film" or "physically at a theater" depending on context?

"At the cinema" normally means physically at the theatre. If you are watching a film at home or on a plane, you are not at the cinema.

If you are at the cinema, you could be in the lobby buying popcorn or you could be in the actual cinema auditorium watching the film. Whichever one you are doing, you are physically at the building known as a cinema.
