Are both (1) and (2) possible? If both are possible, what the difference between (1) and (2)?

Both are possible. There is no difference. (1) is more commonly heard.

The second variety might be heard more when there is more information than "his plans", so that "to make" and "clear" aren't separated by too much. For example:

I have a plan to win the lottery, travel the world and open an animal sanctuary.
I made clear my plan to win the lottery, travel the world and open an animal sanctuary. (Neat sentence)
I made my plan to win the lottery, travel the world and open an animal sanctuary clear. (Not so neat with "clear" right after "sanctuary")
