I think this can depend on cotext, and one firm answer given by the test-maker would be silly.

1. Left alone, he wouldn't feel lonely. = Even if(or If) he were left alone, he wouldn't feel lonely.
2. Left alone, he felt lonely. = When(or As=Since=Because) he was left alone, he felt lonely.
3. Left alone, he will cry.= If he is left alone, he will cry.

Yes, in all cases, it would be better to add the other words.
For 1. 'even if' is better, because 'if' could imply that he is feeling lonely now because people won't leave him alone.
In 2. the extra words give different meanings to the sentence, so if they're relevant, use them.
