    The second one would be correct, with one exception. The word "chose" is past tense. In this sentence you need to use "choose."

    Yes, that word is tricky when you miss the one "o," it completely changes the meaning.

    In what situation would a picture be an alternative to a form?

    Whether you use "then" or not really doesn't matter. If "picture" is an item to be chosen on a computer screen, then you should use quotes or something to denote that.

    Click on "picture."
