    Where do you use commas in an address when the address is in a sentence and the sentence continues after the address?

    2, 4, and 5 are correct.
    I'm not a teacher, but I write for a living. Please don't ask me about 2nd conditionals, but I'm a safe bet for what reads well in (American) English.
