use of commas in a long sentence

Could someone let me know if I placed the commas in this sentence correctly?

I am going to adjust these, but because of the history of cardiomyopathy, and the implantable cardioverter defibrillator I am going to have this interrogated to make sure that she does not note ventricular dysrhythmia.

I would rather see a comma after defibrillator than after cardiomyopathy.
