I don't catch the meaning of the underlining sentences. Need help.

the jig is up - Wiktionary

"The jig is up" means that one's adventures in crime have come to an end. Game over.

In this case it appears to be talking about a medical diagnosis. Maybe they are testing for AIDS. If you get a positive result, then your game is over. You will die.
