Joseph went to school. He still had a fever.

I have transformed the sentence as follows:

Although Joseph had a fever, he still went to school.

Is "still" required when "Although" is used?

It's not required. It's optional. It provides more emphasis of the contrast.

Your new sentence is correct English, but 'still' now has a different meaning.

'Were' is correct, though many speakers of BrE would use 'was'.

Although Joseph still had (continued to have) a fever, he went to school.
