    Are all the sentences correct?

    She insisted that he went with him. (but it doesn't mean the same as the previous one. It's a different sort of 'insist', and it's referring to a fact.)

    In Br Eng, where the subjunctive is much less common (some would say moribund), the second one sounds very formal. In Am Eng, it's fine (though I say it as shouldn't ).
