    She insists/insisted that he goes/goi/went with him.

    She insists that he goes with him.

    She insists that he go with him.

    She insisted that he go with him.

    She insisted that he went with him.

    Are all the sentences correct?

    She insisted that he went with him. (but it doesn't mean the same as the previous one. It's a different sort of 'insist', and it's referring to a fact.)

    In Br Eng, where the subjunctive is much less common (some would say moribund), the second one sounds very formal. In Am Eng, it's fine (though I say it as shouldn't ).
