    The form of a letter

    Hi, my teacher asked us to write a letter as homework. The letter should be concerned with complainting about the poor service of some department store. My teacher didn't mention which department store it is. My problem is I don't know how to write the title in this letter. For example:

    May 1, 2013

    (1)
    49 Happy Lane
    Los Angeles, CA 54280


    Dear Sir/Madam,

    I'm writing to complaint the poor service of your department store.....


    Can I put Sir/Madam in the place (1) ?

    As Rover said, we cannot help with homework - that would be very unfair. Write the best letter you can, submit it to your teacher and when you get it back, you can come back to us for help if you don't understand any of the teacher's comments or corrections.
    Remember - correct capitalisation, punctuation and spacing make posts much easier to read.

    OK, I understand now and thanks again. This is what I got from my teacher. I can't write Sir/Madam there, right? I'd like to know what else I can put there except my teacher's correction (the red words):
