Does "always" always follow by base form of verb?

I always take a bath

When I was a child I always took a bath -is this correct?
pls help..

Yes, it is. You are no longer a child, so you are talking about a past habit. Even if you still regularly take a bath, you're talking about what you did in the past.

It's the past. She is now dead, so she doesn't eat any more. One small correction though: "She always ate bananas when she was living."

That depends. Does his mom still scold him? If so, use the present. If not, use the past.

Last year Ben always gone to church

"Last year" is in the past -- last year finished in December. So, as this is the past, logically we need the past tense. (There's not much logic in English, but this is very logical.)

However, the past tense of "go" is not "gone" -- that's the past participle, which is something different. The simple past tense form of "go" is "went".
