    Default obvious?

What are the three mistakes in this sentence?

On February 23, the 45 year old woman found the house and pulled into the driveway of 3301 Blacksmith Avenue.

    1) the hyphens for the age

    2) the grammar (mentioning the house and the driveway)

    3) perhaps the introductory clause

    1- Yes
    2- Seems OK to me
3- It's OK, unless they are insiting on 23rd.
If the sentence exists on its own, then the use of 'the woman' seems strange because we don't know the identity of the woman, so 'a' would make more sense.
