Who did you mark it for attention of ?

Situation:
When we receive delivery in the warehouse, it is normally marked as ,"FAO Mr. XYZ". I have still not received an expected parcel and therefore contacted the supplier and want to ask them what name did they write as For Attention Of. I am not sure how to ask this question and got some of the sentences in my mind as below:

Who did you mark it for attention of ?
Whose attention did you mark it for ?
What name did you put as FAO ?
What name did you mark as FAO ?

They are all understandable. In #1 I'd say '...for the attention of'.

I would say "to the attention of."

I have not seen "FAO" as an acronym. I would mark it "ATTN: Joe Smith."

'FAO' (For the attention of) is common in BE.

Yes, they are all understandable but are they grammatically correct as well, please ?
