    Dear Mr/Ms _____________

I am writing to inform you that I will be taking __________ weeks maternity leave beginning on ___________(start date of leave). I plan to return on ___________ if all goes well and my physician is agreeable.

    Sincerely,

You should find out what your company's requirements are as far as requesting maternity leave - do you need to contact only your supervisor, or the Human Resources Department, etc?

The law in some countries removes the need for Ouisch's second sentence (although it may be thought polite). In EC countries a prospective mother just needs to say 'I'm coming back' (without giving an estimated time), and the employer has to protect the job. On this point, as Ouisch said, speak to the HR/Personnel department.
