Have you tried Dave's ESL cafe? There's a forum for teachers dedicated to Reading. 8)

With regards to your questionnaire, it would be beneficial if you could add in questions such as

--What country do you teach in?
--Do your students speak the same first language? If not, what are their native languages?
--What genre do you find works best with certain native languages? That is, do you find that certain genres work best with certain students, certain levels, ages, certain first languages?
--With regards to reading, what do your students complain about or find difficult? What/Where do you feel those difficulties stem from?
--Do you use alternative genres such as oral tradition, dance, storytelling? If so, why? If not, why?
--Are your students literate in their first language? That is, can they read and write in their first language.

    All the best, :D
