Default usage "Mrs.NAME1 wife of late Mr. NAME2"

do we have to use 'Sri.' along with 'late' ?

ie, Is the usage "Smt.NAME1 wife of late Sri. NAME2" is correct.

eg :- "Smt. Sonia Gandhi wife of late Sri. Rajiv Gandhi" is correct or not.

Or which of the following is correct ?

    1. "Smt.NAME1 wife of late Sri. NAME2"

    2. "Mrs..NAME1 wife of late Mr. NAME2"

    3. "Smt.NAME1 wife of Sri. NAME2(late)"

    4. "Mrs.NAME1 wife of Mr. NAME2(late)"

    5. "Smt.NAME1 wife of late NAME2"

    6. "Smt.NAME1 wife of NAME2(late)"

"Smt. Sonia Gandhi, wife of the late Sri. Rajiv Gandhi"

It is, though you can replace Smt./Sri. with Mrs./Mr. if you wish, or even leave out the titles. It rather depends on how formal you want this to be. Without the titles is an informal structure.

However, you need a comma after her name, and "the" before late. I would not put "late" in brackets. It is not normal English.

Smt. and Sri. are Indian words which can be used only with the name of a living person.
Smt./Sri. has meaning like 'in very good condition' or 'well being' or 'long live' etc.
So we can not use 'Smt.' and 'Sri.' with the name of a late person.
In indian languages we do not use the prefixes/titles, that are used with the name of a living person, to the name of a late person.
Due to this tradition followed in India , it is not nice to use 'Mr.' and 'Mrs.' with the name of a late person in India ( or INDIAN English) .

"Smt.NAME1, wife of the late NAME2" may be the correct usage.

    Grammatically, all are correct.

From the etiquette point of view, with reference to sanatana's post, A and C would not be correct.

Is #D meant to be the option of "[First name][Second name}, wife of the late [First name][Second name], which is also correct?

'Name2' only is the name of a late person. So 'Shri.' can not be used with 'Name2'
'Name1' is live and 'Smt.'/ 'Mrs.' can be used with 'Name1'

Therefore in INDIA, C and D are correct.
    A is not correct.
    Not sure about B.
