[Grammar] ‘made of’ vs ‘made out of’ vs ‘made from’

Is there a rule of thumb to choose the right option in there sentences:
Bread is made from flour, water, and yeast.
Cheese is made from milk and salt.
Or
This dinner table is made of oak.
This furniture is made of chipboard.
Or
They made a path out of concrete.
He made his car out of spare parts.

My search in FAQ failed. I searched for the whole phrase ‘‘made of’ vs ‘made out of’ vs ‘made from’’, I searched for ‘made of’, ‘made out of’, ‘made from’ but did not get any sensible response.
Please help me how to correctly do that search.
