The enclosed form that I had sent to you

I sent a checklist to our customer to fill in. Later I found that I need to add one more item in the checklist. I will send him an e-mail enclosing a revised checklist . Is the following sentence natural?

“Please note that I added item no. 15 “(name of a dcocument)” in the enclosed form that I had sent to you on 25 May 2014. “

There is no need for the past perfect "had sent", use "sent".

How about Please note that I have added item no. 15 “(name of a document)” to the enclosed form that replaces the one I sent to you on 25 May 2014
