Is there any chance for the sentence below to be grammatical?
ex. She would die unless the doctor saved her.

I would imagine that this sentence could describe the real situation of the past event.
That is, unlike unreal-conditionals, "She was expected to die except if the doctor saved her." Is it right?
And the proposition of the unless-clause was unknown to us whether it was fulfilled or not. Am I right on my thinking?

It works (in the right context), and we don't know without more information whether she lived or died.

Then what is the more appropriate to think - the sentence in question is a hypothetical unreal conditional sentence or a real conditional sentence?

To tell the truth, I just saw this example in isolation somewhere else.
So I basically don't know what context is to be used.
The context I contrive to make as best as I can is just as follows;
The sentence in question is the bacakshifted version of 'She will die unless the doctor saves her'.
But I know that this cannot be any satisfactory at all. Would you please suggest some proper and clever context?

I find it extremely unlikely that you simply saw that sentence, all alone, for no good reason, in isolation. If you can't remember the context, then just say so.

I will also be embarrassed if I see a sentence alone floating without any context. But I cannot help what the given fact is.
I would like to assure you that there was really no context at all when I happened to find this sentence.

One way I think this would work is "She knew she would die if the doctor didn't save her."

And in indirect speech:
They told us she would die if the doctor didn't manage to save her.
