Usually "was found to be the case". It means that something was discovered to be true.

To put it in grammatical terms:

While the present passive "is found" is intelligible, it is hard to think of circumstances in which it might be naturally used. One would almost always use the past passive "was found."

Yes.

Newspaper reports last week suggested that Peter Capaldi would be the new Doctor Who. This/It was found to be true this week when the BBC announced formally that the actor would be taking over the role after Christmas.
