The quality of the answer is usually determined by the quality of the question.
If I am asking the name of a friend, I would use Mike's suggestion, "What is your friend's name?.
"What is your friend called?" is possible, but I find "What is your friend named?" unnatural.
The two I have underlined are fine. They refer to the initial giving of the name. The one I have put in bold is not natural.I found the following sentence in the dictionary: "We named our daughter Sarah."... so if this sentence is fine, why can't we ask "What did you name your daughter?" or "What is your daughter named?" (besides the typical question "What's your daughter's name?")?
