I do not know something? What do you not know? What? I do not know how to fly. I do not know that. That. How to fly.
The adverb 'how' relates the objective complement of the verb 'know'.

How do you know how to fly? *You know that(=how to fly) how?
The adverb 'how' in front position relates to the main verb 'know', the other 'how' in the noun phrase to 'to fly'.

    I hope this helps.

Oh, my Chinese students say that phrase all the time, "How to <do something>?", and, as you know, it's not correct.

Use How do you ... ?, as in

    • How do you get to school?
    • How do I turn on your computer?
    • How did they learn English so fast?
    • How does s/he know the pronunciation of my name?

If you want to use "how to", use it inside a sentence:

    • This is how to get to school.
    • You don't know how to turn on my computer?
    • They learned how to speak English in preschool.
    • She knows how to pronounce your name because I told her how to say it.

    All the best
