Post Please correct my insurance claim letter

This is to inform you that Mr. X has a General Comprehensive Policy of insurance with your company.
On [date], Mr. X rushed to his home town as his father admitted to the hospital in very critical condition. On [date], his father died due to the critical illness.

As per the company policy, we have enclosed the copy of the death certificate, copy of the travel tickets and copy of the insurance policy for your kind perusal.

Therefore, in accordance with terms and conditions of his insurance policy, we would be grateful if you could arrange for the cash grand for travel fare at the earliest.

If you require any additional information, please do not hesitate to contact us.

Thank you in anticipation of your co-operation.

Omit the first sentence and second sentence-you wouldn't be writing if there wasn't a death; the second sentence is unnecessary. Be sure to include the policy number and the holder's full name as well. The remainder sounds fine. However, it can go in one paragraph. Combine the remainder of the information in another paragraph.
