    I would say "concerned about your health" because:
    concerned about = worried
    concerned with = interested, fascinated

    When you mean that you are worried about someone's health i think you should use concerned about.

    I think "for" here is used in its older sense of indicating cause or reason: Stolen kisses, the sweeter for the theft....

    So 'I'm concerned for your health' means my worry is due to your health.
