When addressing an envelope to include all family members, when the married woman has kept her maiden name, her children use the father's last name, and the woman has remarried; [children live with mother, stepfather and keep their natural father's name].

If A is the mother's maiden name, B is the new husband's name, C is the children's natural father's name, is it correct to label as follows:

    A-B-C Family
    1234 Anystreet Avenue

    Re: Anne

For simplicity, use the husband's surname: The A family

    Re: Anne

Or you could say "Mary Anderson and Family"
I'm not a teacher, but I write for a living. Please don't ask me about 2nd conditionals, but I'm a safe bet for what reads well in (American) English.
