    Which one is correct?

    1. I forget / forgot send you the schedule.
    2. The certificate is / has been requested in the 2nd LC.

    I forgot to send you the schedule.
    (You might use forget, but it would usually be in the form of such a phrase as "I tend to forget to..." or "I often forget to..." or "I might forget to...")

    The certificate has been requested...
