    Can we say:

    -How many times have you been to England? (Can we also say, how many times did you go to England?)

    -Why have you been there?

    It depends on the context. If you want to know how many times in total, in someone's entire life, they visited England, then:

    How many times have you been to England?

    However, if you are enquiring about a specific period of time, you would say:

    How many times did you go to to England in 2004?
    How many times did you go to England last year?

    If you are enquiring about the reason for the trip(s):

    Why did you go there?
