    In reported speech, do we always have to change all the tenses. In this sentence, i see that the person uses present tense - is that correct.

    He said he had problems getting a good night’s sleep and he attributes this to his medication.

    No, it's not always mandatory. But it is the custom.
    You'll note that 'had' in "He said he had problems" is past tense, even though 'attributes' is present tense. This sentence is acceptable.
