I am writing a description of a booking software and I would like to write these three sentences under some screens. Thanks you verrrry much

1- (a pic of log in )
We are able to enter to the program by typing the user name and the password.

2-
This screen contains patient's data and by inserting a file number we can see a list of registered patients.

3-
This screen enables us to search by patients ID or their phone numbers.

1. Enter your username and password to access the program.
2. In this window, you can view [patient data][a patient's data - if only one patient]. Insert a file number [so that user instructions are easy to follow it's best to say where, eg 'in the File Number box/field'] to see a list of registered patients.
3. In this window, you can search [for - in original you write 'by' but do you mean 'for'? Don't you know the patient's name and want to find out what their ID and telephone number are?] a patient's ID or contact telephone number.
