It is I who am/is to blame.

Should I use 'am' or 'is'?

I don't know. But maybe you could use, "I am the one to blame"?

'It is I who am to blame' would be correct.

However, this is a very formal construction which we would not use in everyday conversation.

I'd say 'I'm to blame' or 'It's my fault'.
