    Is it correct and needs some correction.
    I phoned to Dr.John to inquire her operation date . He has infromed me that he has not met her personally nor I given her any time frame for operation but saw her medical report. Somebody showed the report and informed me that the patient is out of city.

    Not a teacher only a native.

    From what I can gather from your text, I would write it like this:

    'I phoned Dr. John to enquire about the date of her operation. He has informed me that he has not met her personally, nor had he given her any time frame for the operation, however he has seen her medical report. I have been shown the medical report which says that the patient is out of the city.'

    I think this may be right, it is hard as I cannot work out exactly who is saying what to who. Please let me know if my interpretation is wrong and I will try and correct it.

    I respectfully suggest something like:

    I called Dr. John to ask when the operation would be performed. He told

    me that he had seen her medical report, but he had not yet met her or

    decided on a date for the operation. I learned that the patient is out of

    town.
