I want to issue a warning letter to an employee- pleae see below.
We have received several complaints regarding your poor work performance and failure to complete assigned task in a timely manner and misbehavior to the supervisor and co-workers.
On .... you did not report on duty but your clock card found punched by someone else which is a very serious offence you committed with your knowledge. Refusal to follow supervisorís instructions will not be tolerated and there must be significant improvement in your attendance and specifically, you must treat your supervisor and co-workers with appropriate respect.
Please be advised that if immediate improvement in your behavior is not realized, you will be subject to further disciplinary action that may lead up to termination of your employment.
