    His life has been off track for a while.

    Does it mean his life is going in the wrong direction?

    Perhaps not in the wrong direction, but not exactly in the right direction. Not moving efficiently in the right direction, at least! So it doesn't mean he's entered a life of despair and horrors, but he's not making progress on his goals or what he wants from life.
    I'm not a teacher, but I write for a living. Please don't ask me about 2nd conditionals, but I'm a safe bet for what reads well in (American) English.

    Think of a race track. A car that's not on it (but on the grass beside it) will still make progress, but being on it is better.
