    writing a formal letter

    sir/mam

    i am very confused what/how to write a letter,
    i have to write a letter to the deputy director of
    ICAI about retifying the mistake done by there dept.
    in the name on admit card of Common proficency Test
    of Chartered Aaccountant
    please give ur suggestion on this

    Assuming that it is ICAI who made the mistake, you could write:

    Mr A Yyyyy
    Deputy Director
    ICAI
    address

    Dear Mr Yyyy

    There has been a mistake in the name on the admit card* which I enclose with this letter. The name should read 'Aaaaa', not 'Aaaba'.

    Please send me a card with the correct name.

    * Are you sure it's called an 'admit card'?
