Please consider below senario for my question.

I sent on document to my colleague. He said that the name of document was not correct ?

I asked below question to him . Please check it grammatically .

"What should be the correct name then ?"

What should the name of the document be?

Note that this means you have titled the document incorrectly. It doesn't mean you sent the wrong document, and he was looking for a document with a different name.
